Welcome to the Nurse's Office

My goal is to provide care for each student and maintain a healthy school environment. In order for me to do that, I need the parents' help. Parents are being asked to notify the nurse of any medical problems students may have and keep up-to-date immunization records. This will aid in providing efficient case management of your child(ren). Let's work together and make this a great year!
Keep In Mind.......
It's State Law that all medications be brought to school by an adult in it's original container from the pharmacy and kept in the nurse's office.

Prescription medication will only be dispense by the school nurse or an authorized and trained staff member.

Each school year an annual renewal medication form must be filled out by your doctor in order for your child to take medication.

A medication form must be signed by the parent/guardian.

Please notify us immediately if your contact/emergency number change.

If your child has a doctor's appointment, please keep me informed.

Please ensure that you have filled out the student emergency.

In most cases as needed medications should be given at home, with the exception of Metered Dose Inhalers (MDI's) for Asthma. This still requires a doctor’s order and a permission form completed and signed by the parents.

Teachers must send the student to the nurse with a written pass which includes the name of the student and teacher as well as contact name and telephone number if possible.

A student that is going home due to illness or injury can only be signed out by the contact persons listed on file. Parents, please check in at the front office and remember to bring proper ID when signing your child out of school.
Immunizations need to be current with a (copy) of your child’s shot record on file.

A student that is returning to school from absence due to illness or injury should bring a written statement from the parents or a doctor stating the reason for the absence, the condition of the student.
